Abstract:	研究目的：探討在慢性腎臟疾病營養衛教門診中，營養師執行營養照顧過程與模式(Nutrition Care Process and Model，NCP&M) 其中營養介入(Nutrition Intervention)時病患的行為改變的階段、學習意願及監控與評值(Nutrition Monitoring And Evaluation)之情形，藉以瞭解營養衛教的適切性，進而提昇病患對疾病認知與自我照顧能力。研究對象：以南部某區域教學醫院之慢性腎臟疾病共同照護網，門診收案於六個月中回診一次以上且有執行營養照護一次以上之病人為對象，總計有307位收案慢性腎臟疾病病人，符合條件有281位病人。研究結果：行為改變階段最多數為沉思期，次之為沉思前期占總數80﹪以上，在營養介入時病患學習意願半數以上為勉強，衛教後評值病患對於營養衛教瞭解程度總數71.2﹪為部分了解。
